Targeted Nursing Services
Weekly RN-Led Wellness
THIS IS A WEEKLY SERVICE THAT IS DESIGNED TO KEEP YOU OR YOUR LOVED ONE OUT OF THE HOSPITAL LONG TERM. THIS IS A DROP-IN SERVICE DESIGNED TO FIT YOUR INDIVIDUAL NEEDS.
Weekly RN visit Includes:
Vital Trend Analysis: We don’t just take a BP reading; we look at the week-over-week trends. If we notice a slow, steady climb or a dip, we have the data ready for the next scheduled doctor's visit.
Communication Management: We act as a professional filter. Instead of weekly calls to the office, we compile a Monthly Clinical Summary for the physician. Of course, if there is a "Red Flag" change, we notify the office immediately with a concise, nurse-to-nurse report.
Preventative Health Screenings: Proactive assessment for the common causes of hospital readmissions (like fluid retention or signs of infection). Our goal is to catch the "whispers" before they become "screams."
Integrated Medication Review: We perform a weekly audit of the med-box and any new prescriptions to ensure there are no gaps in compliance or confusion with pharmacy refills.
Functional Stability Tracking: A weekly check on mobility and cognitive. This helps families make informed decisions about long-term independence based on professional data, not just "gut feelings."
Clinical Resource Coordination: We handle the "background noise" of care, such as managing medical supply inventory and ensuring the home environment remains optimized for recovery.
The Targeted Nursing Intervention
HOSPITAL-GRADE PRECISION FOR YOUR MOST CRITICAL AT-HOME CLINICAL NEEDS.
High-level recovery requires more than just "checking in"—it requires a professional to maintain the integrity of your health through targeted, RN-led interventions. This focused encounter is designed to execute complex clinical protocols with total accuracy, ensuring your recovery remains on track and your environment remains stable.
The Clinical Pillars of this Encounter:
Advanced Clinical Surveillance: A high-level physical assessment designed to identify early markers of infection or subtle shifts in baseline stability before they escalate into an ER visit.
Aseptic Wound & Skin Management: Expert care for surgical incisions, complex wounds, or pressure sores using strict aseptic techniques to ensure a sterile healing environment.
High-Acuity Technical Tasks: Professional management of physician-ordered tasks, including staple/suture removal or the management of specialized medical technology (G-tubes, Foley catheters, or IV ports).
Drain & Device Optimization: Expert oversight of surgical drains or medical devices, including output tracking, site care, and real-time troubleshooting to ensure functional integrity.
Biometric Stabilization Audit: A targeted check of vital signs and localized circulation to verify systemic health and healing progress.
The "Skills Lab" Coaching: Hands-on validation for caregivers, teaching them to distinguish normal recovery trends from clinical "Red Flags," moving your family from uncertainty to mastery.
The Recovery Alignment: 4-Hour Intensive
ARCHITECTING YOUR RECOVERY ENVIRONMENT FOR CLINICAL PRECISION AND LONG-TERM SUCCESS.
High-level recovery doesn't always require an all-day presence—it requires a professional to calibrate the home environment to the physician’s clinical standards. This focused, 4-hour clinical residency is designed to "reset" your recovery space, eliminate complexity, and empower your care team with total clinical confidence.
Your 4-Hour Recovery Calibration Includes:
Targeted Clinical Surveillance: We perform a high-level, multi-system physical assessment and vital signs trend to establish your post-operative baseline and verify that your healing trajectory is on track.
Medication Safety & Reconciliation Audit: We perform a professional audit of all prescribed medications, reconciling hospital discharge orders with your home history to ensure total dosing accuracy and safety.
Clinical Competency Validation: We provide focused, professional instruction on specific medical tasks (such as drain care, sterile dressing changes, or specialized equipment). We don’t just "show" you; we validate your caregiver’s technique so they can proceed with total confidence.
Recovery Architecture & Optimization: We identify clinical bottlenecks in your recovery space—optimizing mobility paths and organizing medical supplies into a professional "Command Center" to minimize the risk of falls, infection, or medication errors.
The 24-Hour Strategic Protocol: We leave you with a written, hour-by-hour clinical strategy for your next 24 hours of recovery, removing all guesswork regarding medication timing, hydration goals, and "Red Flag" symptom monitoring.
The Medication Safety Reset
TRANSFORMING PILL-BOTTLE CHAOS INTO COMPLETE CLINICAL CONFIDENCE.
Are you or a loved one feeling overwhelmed by confusing doctor instructions, duplicate prescriptions, and a cluttered medication cabinet?
Managing multiple prescriptions is one of the highest risks for seniors aging at home. At Triad Nurse Concierge, we step in to eliminate the confusion, ensure clinical safety, and hand you back a perfectly organized system that is safe and easy to manage.
This is not just filling a plastic box. This is a comprehensive, RN-led clinical audit designed to catch dangerous interactions and establish a safe baseline for your health.
In Person Appointment Accompaniment
STRATEGIC PRESENCE & COMMUNICATION FOR HIGH-STAKES CONSULTATIONS.
Navigating a 15-minute specialist appointment can be overwhelming. We provide a professional clinical presence to ensure your history is accurately represented and your future treatment plan is crystal clear. This service is designed to maximize the value of your time with your physician—whether that appointment happens in a clinic or via a telehealth screen.
What the Service Includes:
Expert Pre-Visit Strategy: A deep-dive review of your current symptoms to develop a Targeted Topic List for the doctor.
Flexible Clinical Presence: We join you as your professional advocate and clinical "translator," either in-person at the clinic or by remotely joining your telehealth/virtual consultation.
Real-Time Clarification: We bridge communication gaps during the live call or visit, ensuring that "doctor-speak" is translated into actionable information immediately.
Professional Post-Visit Summary: You receive a Plain-Language Strategy Note in your portal, summarizing new orders and exactly what your next steps are.
In Person Appointment Acute Hospital Advocacy
PROFESSIONAL CLINICAL OVERSIGHT & LIAISON SERVICES DURING HOSPITALIZATION
When a loved one is hospitalized, the system moves fast and families often feel left behind. We act as your private Clinical Liaison, monitoring the hospital's plan of care from the "outside" to ensure safety, continuity, and a clear path toward discharge.
What the Service Includes:
Clinical Record Surveillance: We review hospital notes and results via your patient portal to monitor trends and recovery progress.
Physician & Staff Liaison: We act as the central point of contact for the medical team, ensuring your clinical history is accurately understood by the rotating hospital staff.
Bedside Advocacy Visits: Focused on-site visits to attend "Physician Rounds," ensuring your family's questions are addressed and the discharge plan is being safely prepared.
Discharge Readiness Audit: We review final discharge orders before you leave the building to ensure medication and follow-up care are perfectly aligned with your needs.
Targeted Dignity & Personal Care Visits
FLEXIBLE 1-TO-2 HOUR CARE DESIGNED TO FIT YOUR SPECIFIC NEEDS
Traditional caregiver agencies do extraordinary work providing dedicated, multi-hour companionship and daily living assistance. However, when your loved one doesn’t need a companion for half the day—and simply requires a safe, dignified bath once or twice a week—we offer focused 1-to-2 hour personal care visits led directly by an experienced Registered Nurse.
WHAT’S INCLUDED IN EVERY VISIT:
Assisted Bathing & Hygiene: Respectful, dignified bed baths, seated showers, hair care, and hygiene routines paired with expert, low-fall-risk transfer techniques.
Simultaneous Clinical Skin Audit: While providing care, an RN actively inspects high-risk pressure points, surgical sites, and fragile skin to intercept breakdown before it develops into a chronic wound.
Mobility & Fall Safety Screen: Continuous evaluation of dynamic balance, transfer stability, and home environmental hazards during actual movement.
Baseline Vital Check & Summary: Comprehensive vitals screening and an immediate visit note uploaded to your family’s secure portal.
IDEAL FOR:
Seniors who only need focused, professional hygiene or shower assistance without requiring half-day or full-day home supervision.
Individuals with mobility limitations, Parkinson’s, stroke recovery, or joint replacements who benefit from skilled clinical transfers during hygiene routines.
Hospice Support, End-of-Life Extended Respite
CLINICAL BEDSIDE CARE & FAMILY RELIEF
Routine hospice visits are brief and intermittent, leaving families to manage around-the-clock symptom crises, complex dosing schedules, and severe emotional exhaustion on their own. We bridge the critical gap between routine hospice check-ins by placing an experienced Registered Nurse directly at the bedside—ensuring uninterrupted comfort for your loved one and restorative rest for your family.
What’s Included in Every Session:
Continuous Symptom & Comfort Management: Precise, timely administration of scheduled and breakthrough comfort medications (for pain, dyspnea, anxiety, and terminal restlessness) strictly adhering to your established hospice orders.
Dedicated Extended Family Respite: 4- to 8-hour daytime, evening, or overnight blocks so primary caregivers can sleep, attend to personal obligations, or step away with complete peace of mind.
Compassionate Transition Vigil: Gentle, dignified bedside presence providing continuous clinical reassurance and helping family members navigate the physical stages of the dying process.
Positioning & Airway Comfort: Expert repositioning, gentle oral care, secretions management, and non-pharmacological comfort interventions to maintain continuous peace and dignity.
Ideal For:
Families facing the exhausting physical and emotional demands of round-the-clock end-of-life care.
Loved ones experiencing breakthrough pain, agitation, or changing symptom profiles that require continuous clinical monitoring.
Family caregivers who need reliable overnight or daytime relief to prevent severe caregiver burnout.
